Thank you for choosing Bedminster Orthopaedics & Sports Medicine. To make sure your visit runs smoothly, you are required to bring the following information with you:
Print and complete Patient Intake Form
Insurance Cards – Primary and Secondary
Insurance Referral (If required by your insurance carrier)
Driver’s License or other form of identification
X-rays (If done by another doctor/facility of body part being evaluated regardless of findings)
MRI Films and Report (For body part being evaluated regardless of findings)
CAT Scan/Bone Scan Films and Report (For body part being evaluated)
Operative Report (If body part being evaluated was operated on by another surgeon)
We encourage you to bring your own shorts, if doctor will be evaluating your knee or hip.
If you are being seen in the first 10 days following the accident, no authorization is required
If you are being seen after that 10 day period, your adjuster must fax a letter of authorization to 908-234-2022. This letter should include:
Name of adjuster, telephone and fax numbers
Claim number
Claim address
Statement that patient is authorized to be seen in our office (This is our office policy)
