Print and complete Patient Intake Form Insurance Cards – Primary and Secondary Insurance Referral (If required by your insurance carrier) Driver’s License or other form of identification X-rays (If done by another doctor/facility of body part being evaluated regardless of findings) MRI Films and Report (For body part being evaluated regardless of findings) CAT Scan/Bone Scan Films and Report (For body part being evaluated) Operative Report (If body part being evaluated was operated on by another surgeon) We encourage you to bring your own shorts, if doctor will be evaluating your knee or hip. Copays are expected to be paid at the time of your visit.

Failure to bring any of the above items will result in the rescheduling of your appointment
